Flames Team Uniform and Club Fee Credit Card Payment

This form is for coaches only to order uniforms and use a credit card for payment.
 Any  individual player payments will not be accepted.

Team Fees:

1. Club Fee is $50 per player, due in Fall and Spring each year. 
   In the Fall it is only for our 13U and below, in the Spring it is for every team including the high school teams as well.
2. Uniforms:  Girl’s Dazzle $130 complete set home and away.  Pants are $25 each, Jersey’s are $40 each.
3. Uniforms:  Boy’s Mesh $75 complete set, reversible jersey.  Pants are $30, Jersey is $45.
4. Shooting Shirts: $45, include players last name, three styles, all White, White with Red Stripe, White with Black Stripe
5. Shooting Pants: $40, includes players number, Black Only.
